Accounts are billed 5^TH Day of each month and due and payable upon Receipt

All Disputes must be filled within 7 days

the Undersigned authorizes and concents to the receipt and exchange of credit information and agrees to abide by thetermssetout in this credit agreement.

To terminate the account a written notice is required
